Nothing covers them, nothing is applied to them, and they face the sun at a high angle for the whole session. They are the most consistent gap in any exposure plan.

Lips and ears fail for a different reason from everywhere else. Nothing removes protection from them, because nothing was ever applied. They face the sun at a high angle, no standard kit covers them, and they sit outside everybody's mental image of where sun protection goes. A brim handles the ears, and a dedicated lip product used at every stop handles the lips.
Go through the exposure maps on this site and one pattern repeats in every sport. Two rows have nothing in the removal column, because nothing was ever there to remove. The ears and the lips are not failures of a plan. They are omissions from it.
It is worth understanding why, because the reason is the same in every case and the fix follows from it. People apply protection to the surfaces they can see in a mirror while thinking about sun: the face, the arms, the shoulders. Ears are at the edge of that mental image and lips are outside it altogether, filed under something else entirely.
The top rim of the ear faces upward and outward, which puts it at a high angle for the whole of any outdoor session. It sticks out from the head, so it is unshaded by hair for most people. A peaked cap does nothing for it. A helmet does nothing for it. Sunglasses arms sit across it and rub. And it is almost never treated.
The result is that the top of the ear is one of the most consistently exposed surfaces on the body in every land sport, accumulating a load session after session for years. It is a surface that outdoor workers, sailors, farmers, cyclists, golfers and hill walkers all present to the sun continuously, and one that almost nobody plans for.

Lips are the other consistent omission and they fail differently. They are presented forward and slightly upward. They have very little of the pigmentation that other skin has. And the removal rate is high: drinking at every stop, eating, wiping, licking, wind drying and a gel wrapper going past them all take whatever is there.
The lower lip is more exposed than the upper, because it faces forward and up while the upper lip is angled down and partly shaded by the nose. It is a small enough surface that people dismiss it, and yet it is the one that most reliably ends up sore after a long day on the hill, a round of golf in wind or a summer on a bike.
The control is unglamorous: a dedicated lip product with ultraviolet protection, carried somewhere reachable, used at every drink stop rather than when you remember. The two habits reinforce each other, which is what makes it survive a long session. A lip product in a bag in a car is a lip product that does nothing.
Swimming guidance rarely mentions lips at all, because immersion and towelling dominate the conversation and a lip product is a minor detail in that context.
In land sport the lips are exposed for the full duration, dried by wind, and stripped several times an hour by drinking and eating. It is a duration and repetition problem, and it needs a product you carry rather than one you applied before setting off.

The reason these get missed is that they are not part of anybody's application sequence. The fix is to change the sequence rather than to try harder to remember. A sequence that works for most people, done at home before dressing:
Done in the same order every time, it becomes automatic within a fortnight, and the two surfaces that used to be missed every session stop being missed at all.
Both ears and lips are sites people are told to keep an eye on, and both are easy to overlook because you cannot see them well. Anything that is changing, not healing, persistently sore or simply worrying you is a question for a GP. Cancer Research UK and the British Association of Dermatologists publish plain guidance on what to look out for. We do not assess anything here.
Lips and ears are the clearest evidence that exposure planning fails through omission rather than through effort. Nobody neglects them because they decided the risk was acceptable. They neglect them because those surfaces were never on the list, and a list nobody wrote down is a list with holes in it. That is the entire argument for running an exposure map once, properly, and writing it down.

The top rim faces upward and outward for the whole session, which makes it one of the higher-angle surfaces on the body.
A plain balm moisturises but does not necessarily provide ultraviolet protection, which is a separate property. A product that states ultraviolet protection is doing a different job from one that simply softens. We do not recommend products, so the point is to check what the product claims rather than to assume.
More often than anywhere else, because drinking, eating, wiping and wind all remove it several times an hour. Attaching it to drink stops is the habit that survives, because it happens anyway and needs no separate memory.
Most peaked caps do not. A wide brim does, and a cap designed with genuine ear coverage or a flap does. If your headwear does not reach the ears, they need a deliberate application step instead.
