People remember the one scorching day. The total comes from four ordinary sessions a week for five months, none of which felt like anything at all.

The largest part of an athlete's annual exposure does not come from the memorable hot day. It comes from four or five ordinary sessions a week across five months, each of which felt unremarkable. Because the pattern is routine rather than exceptional, the effective response is a structural decision applied to every session rather than vigilance on the days that look dangerous.
Ask somebody who trains outdoors when they last had too much sun and they will name a day. The sportive in the heatwave. The tournament in June. The walk that went on longer than planned. It is a real event and it is not where the total comes from.
The total comes from the sessions nobody remembers. Four or five a week, an hour or two each, from March to September, in ordinary conditions that never announced themselves. Nothing about any individual session was remarkable, which is exactly why no plan was made for any of them, and why the sum is so much larger than the memorable days.
It is worth doing the sum for your own training, not as a scare but as a planning exercise. Take your typical week in season, count the hours spent outdoors, and multiply by the number of weeks between the clocks going forward and the end of the summer. Most people are surprised by the answer, and most people find that the exceptional days account for a small fraction of it.

Planning for the exceptional day is a reasonable instinct and it produces a plan that works perhaps five times a year. The other hundred and fifty sessions happen with no plan at all, because nothing about them triggered one. That is the failure mode, and it is not a failure of care or knowledge. It is a failure of the trigger.
The alternative is not to be more vigilant, which does not scale, but to make one structural decision that applies automatically to every session without needing a trigger. That is what everybody who handles this well has actually done, whether or not they would describe it that way.

Each of these takes one decision and then never needs deciding again. That is the property that makes them survive a season, in exactly the way that a resolution to be careful does not.
The swimming version of this problem is easier, because the sessions are bounded, the venue is usually indoors in this country, and the exposure events are discrete and obvious.
Land training has no discrete events. It has a continuous background of ordinary sessions that nobody registers, which is why the response has to be a default rather than a decision.
If you walk or cycle to work, that is two exposures a day, at fixed hours, for the entire year, and almost nobody includes it in any mental tally because it is transport rather than training. The morning commute is usually at a modest hour and the evening one likewise, which helps, but the sheer repetition makes it a significant share of an annual total for anybody who does it.
It is also the easiest thing on this list to fix, because it is the same route every day, at the same time, in the same kit. One decision about sleeves and a cap, made once, applies to four hundred trips.
Athletes review training blocks as a matter of course: what worked, what was too much, what to change. Adding one line to that review costs nothing and produces the only feedback loop available here. Was anything sore. Which surfaces show a line. What was never treated once in five months. Three answers, and they become the structural decisions for the next block.

The people who manage this well are not more careful than anybody else. They have simply moved the decision from the day to the season, which means it gets made once, when they are sitting down and thinking clearly, rather than a hundred and fifty times in a hallway on the way out of the door. That is the entire trick, and it is the same one that works for every other part of training.
